At Pathways, we know that moving into a new home can feel daunting. That’s why our priority is to create a home where children feel safe, understood, and supported from the very first day.

Every child is unique, and every care plan reflects that. We look beyond challenges to see the whole child – their strengths, potential, and ambitions. By building on these, we create the right environment for trust, stability, and growth.

Our homes are warm, inclusive, and nurturing places where children can feel a true sense of belonging. This isn’t a “placement” – it’s about providing a safe place from which children can rebuild confidence and look forward to brighter futures.

Who We Care For

We welcome children aged 12–17 with a wide range of needs, including:

  • Social, emotional, and behavioural difficulties
  • Challenging or offending behaviour
  • A history of going missing from home
  • Moderate learning difficulties, including autism and ADHD (where not a primary care need)
  • Concerns around child exploitation
  • Self-harming behaviours

We also support children who have experienced:

  • Family difficulties
  • Multiple placement breakdowns

Every referral is risk assessed in partnership with the Local Authority and wider multi-disciplinary team.

How We Work With Every Child

  • Tailored care planning – children are actively involved in shaping their care and support, helping them take ownership of their journey.
  • Dedicated key workers – every child is matched with a key worker who builds a trusted relationship and provides consistent guidance.
  • Education and development – access to education from qualified teachers and support through our in-house education team. Functional skills, GCSE support, and accredited training programmes all play a part.
  • Independence skills – structured support in cooking, budgeting, laundry, and personal organisation, with opportunities to complete the ASDAN Living Independently Short Courses.
  • Cultural and religious needs – respected and woven into daily life, ensuring every child feels recognised and valued for who they are.

Our ambition is simple, to give children a safe and supportive home where they can rediscover trust, develop resilience, and move towards a positive, independent future.

We know progress doesn’t happen overnight. That’s why we stay consistent, patient, and committed – celebrating every milestone, however small, and helping children believe in what’s possible.

Move-On Support:

Our responsibility doesn’t end the day a young person leaves Pathways®.

We:

  • Support carefully staged transitions into independence or new placements.
  • Share insights and outcomes with Local Authorities, evidencing the difference our homes make.
  • Keep doors open for young people who want to stay in touch — because relationships built here matter.

Progression Into Independence:

For some young people in our care, moving into our Transitions supported living accommodation at 16 can be a positive next step. However, this is never assumed or automatic. While we would love to continue supporting young people as they grow, every transition is planned in partnership with the young person, their Local Authority, and the wider professional network to decide what is truly best for their needs.

When supported living is the right option, we take time to prepare the young person at their own pace, ensuring they feel ready for greater independence.

This approach allows them to:

  • Build confidence and self-belief.
  • Practise essential life skills in a safe, supportive environment.
  • Experience independence with the reassurance of continued guidance.

For others, a different pathway may be more appropriate – and we will always be open and honest about that. Our priority is making sure every young person is supported into the right environment for their future.

Where supported living is the chosen route, Transitions® provides the stability, encouragement, and opportunities young people need to step into adulthood with security and hope.
