At Transitions LD, we believe every young person deserves more than just a safe place to live – they deserve a home where they feel comforted, respected, and able to enjoy the best possible quality of life. A place where they can experience joy, build meaningful connections, and live life to the fullest.

Our services are designed for children and young adults aged 16–25, combining specialist care with flexible, personalised support. We take time to understand each individual, building a plan that reflects not just their needs, but their strengths, aspirations, and potential.

Where a young person may struggle to express their wishes, we work sensitively – listening, observing, and collaborating with families and professionals to make sure their voice is heard in ways that feel right for them.

Who We Help

We support children and young adults aged 16–25

This includes:

  • Severe or moderate learning disabilities (including autism and ADHD)
  • Social and emotional difficulties
  • Challenging behaviour
  • Self-harming behaviours
  • Histories of family breakdown or placement disruption

Our Homes:

We know that no two young people need the same environment to thrive. That’s why we offer two types of accommodation:

  • Care Suites – self-contained, private spaces for those who need or prefer their own environment.
  • Shared Homes – welcoming, community-based settings for those who benefit from living alongside peers.

This adaptable model allows us to support both independence and connection, tailoring care to what feels right for each young person.

How We Work With Young People

  • Individualised support plans – designed around each person’s abilities, interests, and long-term goals.
  • Positive behaviour support – strategies that focus on understanding triggers, reducing risks, and building coping skills.
  • Independence building – support with daily living skills such as cooking, self-care, budgeting, and managing routines.
  • Education and activities – structured opportunities for learning, trips, and experiences that help young people grow in confidence.
  • Collaboration – close partnership with families, schools, health professionals, and Local Authorities to ensure care is consistent and joined up.

Transitions LD

Our Transitions LD service provides tailored support for young people moving into adulthood. We work alongside individuals, families and professionals to promote independence, confidence and positive outcomes through person-centred planning and practical support.
