A Warm Welcome to Transitions

At Transitions, we understand that leaving care and stepping into independence is one of the biggest transitions a young person will face. That’s why our focus is on making this journey feel supported, empowering, and achievable.

Our self-contained accommodation provides young people with the space to live independently while knowing help is close at hand. This balance of safety and autonomy allows our young people to build confidence, develop life skills, and take ownership of their future at a pace that feels right for them.

We believe independence doesn’t mean being left to cope alone – it means having the right support, in the right way, at the right time.

Supported living and independence

Who We Help

We welcome young people aged 16–25

This includes:

  • Young care leavers preparing for independence
  • Children experiencing family breakdown or homelessness
  • Young people with mild learning disabilities
  • Unaccompanied asylum-seeking minors

Every referral is risk assessed in partnership with the Local Authority and wider multi-disciplinary team. We also consider planned moves, where parents arrive before their child, so they can settle and build positive relationships with the staff team.

How We Work with Young People

  • Individual support planning – every young person is actively involved in shaping their own plan, aligning with their Transition’s Plan.
  • Key worker relationship – each young person is matched with a dedicated key worker who provides guidance and encouragement.
  • Practical independence skills – support in cooking, budgeting, cleaning, and managing a household, with opportunities to complete the ASDAN Living Independently Short Course.
  • Education and training – access to qualified teachers, accredited training programmes, and further learning opportunities.
  • Respect for individuality – cultural, linguistic, and religious needs are always taken into account and supported.

Our Goal for Every Young Person

Our ambition is clear: to provide young people not just with a place to live, but with the care, sense of belonging, and the skills, confidence, and resilience to step into adulthood successfully.

We measure success not only in outcomes achieved while with us, but also in the steps our young people are able to take once they’ve moved on – whether that’s further education, employment, or safe and sustainable housing.
